It is important for Clackmannanshire Council to know how you and your relative/ the person you care for feel about the quality of the Short Stay Assessment service at Ludgate House.

We would like you to help us by taking a few minutes to answer some questions on the attached questionnaire. Please post it back to us in the FREEPOST envelope provided. The person you care for will also be asked to complete a questionnaire.

The findings from the research will be used by Clackmannanshire Council to help improve the service.  Your opinions will be looked at along with everybody else's, then we will draw conclusions on the main issues that people are telling us about and write a report.  This will help plan future services and make improvements where required.
